Medications at School
All medicine (over the counter or prescription) administered at school, must have a medication authorization form on file in the office. Prescription medicine must have a physician's signature on the form before medicine can be administered. All medicine must be in its original container, with the child’s name on it. The form and medicine must be brought to school by a parent or other adult, and will be kept in the school clinic.
Students with allergies and/or asthma should have an asthma and/or allergy card on file as well.
Health Treatment Plans/Emergency Treatment Plans
Students who require Health Treatment Plans or Emergency Treatment Plans must have the plans defined and signed by their physician and approved by the appropriate PWCS personnel. The plans must be on file before students may attend school.
